| How Easy Is It? |

| Step One: |
You will be given a booklet that lists providers in your area. For additional providers, you can call your representative number on the back of your membership card or visit our list. |
| Step Two: |
You will be given a description for each benefit covered under the AllCara program. |
| Step Three: |
Upon checking in at the doctors office, you will present your AllCara ID card to the provider at the time of service. Remember, all of your legal dependents can use your membership. |
| Step Four: |
At the time services are provided, the participating provider's staff will call the toll-free number on the card to get the AllCara price for services rendered. This process should take only a couple of minutes and this price will be the new discounted price from the provider's normal charge. The new total cost is determined by an agreement between AllCara and the participating provider. |
| Step Five: |
The remainder of the payment is due at the time of your visit. |
