- Memberships are annual even if billed on a month to month basis. Members may cancel their membership at any time by providing written notice to AllCara Card accompanied with the return of their ID card/cards. Any membership cancelled during the first 30 days will receive a full refund of the membership fee. No refund of $50.00 application fee will be made.
- Enrollee understands that The AllCara Card is a healthcare savings plan, and not an insurance plan. By presenting your AllCara Card to the provider, you will receive a substantial discount off of the provider's regular fee. Actual discounts vary depending on the nature of the service rendered. Only hospitalizations require a 30-day waiting period from membership activation date.
- The AllCara Card reserves the right to terminate an enrollment or deny eligibility in the program for lack of payment to The AllCara Card. Returned checks, insufficient funds notices on bank draft or denials by the member's credit card company for payment of the program fee is deemed evidence of nonpayment by member.
- The AllCara Card reserves the right to terminate membership for failure to pay a medical provider accessed under the The AllCara Card program under the terms provided.
- The AllCara Card provides savings to its members on healthcare services through medical networks. In order to access these networks and related discounts, members and member's dependents must pay the medical providers promptly. Payments are due at the time of service unless prior arrangements have been made.
- The providers listed in the on-line directories are accurate as of the original date of publication; however, they are subject to change without notice. Members may call toll free customer service line for updated information.
- Participating Medical providers are independent contractors. The AllCara Card does not practice medicine or in any manner interfere with or participate/patient relationships. All healthcare decisions are between patient and provider. The selection of a provider is the obligation and decision of the patient and is not based upon credentialing by The AllCara Card, its affiliates or its contracted networks.
